Referring Someone to Greater Care Support

We welcome referrals from participants, families, carers, support coordinators, allied health professionals, hospitals, and community organisations. Our intake process is straightforward, respectful, and designed to move quickly so that people get the support they need without unnecessary delay.

Who Can Make a Referral?

       The participant themselves

       A family member, carer, or advocate

       A Support Coordinator or LAC (Local Area Coordinator)

       A GP, specialist, hospital discharge team, or allied health professional

       A community organisation or social worker

Regardless of who makes the referral, we will always seek the participant's informed consent before proceeding.

What We Need to Get Started

To process a referral, it is helpful to have the following information:

       Participant's name, date of birth, and contact details

       NDIS number (if available)

       A copy of the participant's current NDIS plan or plan summary

       Information about the supports required and any goals

       Details of any existing providers or services

       Any relevant medical, behavioural, or communication considerations

Don't worry if you don't have everything — contact us anyway and we'll help fill in the gaps.

Our Intake Process

Step 1 — Initial Contact

Call, email, or fill in our online referral form. Our intake team will respond within one business day.

Step 2 — Needs Discussion

We'll have a conversation with the participant and/or their support network to understand goals, preferences, and requirements.

Step 3 — Matching & Planning

We'll identify the right support workers and develop a support plan that reflects the participant's goals and NDIS plan.

Step 4 — Service Agreement

We'll prepare a clear Service Agreement for the participant (and/or their representative) to review and sign.

Step 5 — Supports Begin

Supports commence on an agreed date. We'll check in regularly in the first weeks to make sure everything is going well.
